Note: We submit claims to all plans except HMO, Medicare, and Medicaid, as we are not in-network with those programs and they do not provide out-of-network benefits.
Information & Checklist
Please make sure you have completed the following before you arrive:
- Filled out the patient intake form HERE.
- Have a referral from your dentist.
(Wisdom teeth consults and biopsy screenings do not need a referral). - Come with any X-Rays your dentist feels are relevant.
Otherwise, we might have to take one of our own in order to complete the consultation.
Designation & Minors
Minors and those with legal or court-appointed guardians must present with a parent or a legal guardian at all times. A parent or legal guardian is the only legally recognized person able to sign any office, financial, or HIPAA agreements, intake forms, and consents for surgery. If a parent or legal guardian is not present for those requiring them, the appointment will need to be rescheduled.
